Advanced Layer Pop-up doesn't show up in DW CS6

Advanced Layer Pop-up is installed with username and password, but the extension doesn't show up anywhere in CS6.CS6 is installed om Mac OS 10.8.4

I finally did get the extension to show up, but DW crashes as soon as I select the extension
