Parallax Slider won't insert in my classic ASP page

In Dreamweaver CS5.5. I click insert and nothing happens.I'm trying to insert into a template and there is not even an error message.
doctype is <!DOCTYPE html>
Anybody encountered this nad corrected it?

Did you restarted DW after installing the extension.
Can you please try to insert the extension by clicking on the icon from DMXZone toolbar. If you don't have such toolbar:

I restarted DW and the machine after installing.
It works in Designer mode with clicking the icon.
Does this mean I have to switch to designer mode everytime I want to use this extension?
I'll delete the extension and re-install completely and let you know what happens

I downloaded a fresh copy of the extension, deleted the previous download with exrtension manager and reinstalled it.
All seems to be inserting fine, but I don't get the initial dialog box taht allows me to create parameters for the slider.
