Hi,
You shouldn't be using Adobe Extension Manager in order to install the extension. Please use the DMXzone Extension Manager instead.
The problem on the screenshot usually happens, when DW gets updated from cc2014 to cc2015 and then the configuration folder of DW gets corrupted, as it tries to "transfer" extensions to the newer version, which is not possible.
Please rename/delete the DW configuration folder, restart your DW and reinstall your extensions using DMXzone Extension Manager.
