Hello David.
Can you please clear your DW Cache file . After that open blank page, create simple form and apply the validator. Notify me in the thread if the error still appear.

I've cleared the Dreamweaver (CS6) Cache (OS Windows 7), then uninstalled and reinstalled the validator.
The validator still is not working. When I created the simple test form, entered some validation with the validator, and displayed the form, it began with this error message: Warning: session_start() [function.session-start]: Cannot send session cache limiter - headers already sent (output started at /home/content/34/10842834/html/test-form.php:193) in /home/content/34/10842834/html/ScriptLibrary/dmxValidator.php on line 253
Also, the validator's display is skewed (.css issues?). Fields run together.
Many of the validator fields do not work. For example, maximum and minimum list values do not let me add values.
