Can you please try to clear your DW cache file?

Clearing Dreamweaver's cache
Once DW cache file is cleared , try to place the Advanced Multi Uploader again on blank page.

Currently the both extensions are not compatible with each other. Our developers are working on update of the both products. You will be notified when the update is released in the thread.

Having bought the latest version of Pure PHP Upload yesterday (v3.0.2) I'm very disappointed to report I'm having exactly the same issue!
