Pure ASP Upload 3 doesn't work with Bootstrap

I was just working with some dummy pages to learn how to use the bootstrap stuff, as I just purchased the boostrap elements today. I found out right away that Pure ASP Upload does not work on a page using bootstrap and/or the bootstrap elements. Every single time I try to apply the PAU 3 extension I get this error in Dreamweaver:
For the record, after I install any new extension, I always close Dreamweaver and open the config files and delete the winfilecache file.
I am on Windows 8 and running an updated version of Dreamweaver CS6. The page I was trying to load PAU3 to is this one:
testdmxzone.scriptsource.com/fileupload.asp
What's the story here? I'm not doing anything at all complicated and not mixing your extensions with anyone else's.

The only problem that is causing this is, that your form does not have a "name".

You were 100% right. Once I added the form name, everything worked fine.
Here's something you should be aware of though. I used bootstrap elements to create the original form, and it only creates a form ID. For whatever reason, it does NOT create the form name. When you create a form in Dreamweaver, it created BOTH the form name and the ID.
Should the form creation options in bootstrap elements be updated so they do it like Dreamweaver? I think a lot of people will run into problems because forms are missing the name, and both your time and theirs will be wasted on troubleshooting this. I think most people would assume your extensions, being designed for Dreamweaver as they are, would create form tags just like Dreamweaver does so you don't run into issues like this.
