HTML Order Form

Is there an extension in the works for an html order form where basic real-time shipping calculator can be included and the form results emailed to the specified email address in a tab delineated file?

Adv Layer Popup 1.06

I've downloaded and installed the Adv Layer Popup update to 1.06. When I try to access it in Dreamweaver CS3, it still shows version 1.05. 

Any ideas?

I am desperately seeking a simple way of adding a visitor poll on my site similar to the poll at the bottom os this page.

I have tried many different poll systems and not seem to work without extensive additional coding.

I would like to see an extension that will allow me to build, customize and implement a completely functional Poll on a webpage without having to do any of the coding.
