automatically updating form fields from database

Hello I need to build a site that people can use to submit problems they have with their IT.  I need to make the submitting form as fool proof as possible.

I want to be able to use dynamic form fields that obtain data from a MySQL db and when one field is filled it automatically fills the next using data from a different table.  I am using a building field that when selected, updates the room field with the corresponding room names.

I am a very beginner with both ASP and Dreamweaver as i have only been using it a few days.. please help.
