Creating an email questionnaire in DW

I've created forms with help from a friend, but he copied and pasted hidden commands from another form to the form I created online. (Example: http://www.ceai.org/forms/brochures/list.htm)

Now what I need to learn is how can I create a questionnaire that I can email to clients?  My goal is to provide yes/no buttons to answer questions directly on the email they receive and provide a place for them to enter contact information (name, address, etc.) in fields there on the email.

How do I pre-edit photo navigation pages?

I am designing a website that contains hundreds of images, so using the "Create Web Photo Album" command has come in quite useful. I make sure the "Create Navigation Page" box is checked so my users can surf from one photo to the next, or be able to get back to the index. My problem is that the photo navigation page that Dreamweaver creates uses default text, left-justifies the image and the text, and uses white as the background color. How do I pre-format this navigation page to suit the look of my site?

DW MX2004 ASP Membership App

I would like to get a tutorial on how to make a membership application, specifically where a new user registers and then the record is inserted into the db but with a flag that says the membership is inactive.

It then extracts the new record from the db, emails the administrator and includes an encrypted link to a confirmation page, which when opened with the encrypted link takes the URL parameter and updates the db to make the user 'active'. It then should email the new member telling them that their membership is now active.

I would like this to be based around existing the server behaviours, but I also have a few 3rd party ones (incl. Ultrasuite) so don't mind if they are used. I want to make sure that I am not leaving myself open to SQL injection attacks.
