Dreamweaver MX Extensions

Creating Your Own Extensions for DreamweaverMX As far as I can tell, this is the only published book strictly on extending dreamweaver. While a few things have changed between MX and MX2004, all the examples still work and with the extending dreamweaver reference in help and on the DW install cd, it is not very hard to make the minor changes to the code for it to fit the MX2004 model. There is not much you would want to do with DW extensions that you could not learn from this book.
