How do I get my form to send info to the database

I need help getting Dreamweaver dynamic pages to send info to my database. I have created a registration form using DreamweaverMX which is in PHP. All the database connections are functioning and I can see the database tables in DW. I can even get the page to show the database info so I know the connection is working. But when I try to 'submit' the form nothing appears in the database at all. I've checked through the DWMX tutorials and according to that i'm doing it right. I've looked at each and every text field, labeled it and tally'd it up to the db. So where am i going wrong?

The database is mySql on my hosting company. Thanks!
