TO INSTALL AN EXTENSION (i'm not sure but i think this is what you're looking for) open extension manager, it has a circular blueish/purple icon. a dialogue window should appear named "Macromedia Extension Manager" with two main sections and a drop down menu. Select Dreamweaver from the dropdown menu. from the File menu select FILE > INSTALL EXTENSION. you will be prompted to navigate to your desired extension. once you select your extension and hit OPEN, you will probably be prompted to accept or decline their licensing agreement, then the extension is installed. if you select the extension in the top window, you can read more information in the bottom window about the extension and how it works. however if the above does not work, your extension may not be compatible with your system or your version of Dreamweaver. not all extension work on both mac / pc, and not all extensions work with newer versions of Dreamweaver. thanks, rebecca

I'm tracking with you James,  I just spent almost 2 hours as well.  And, I consider myself an intermediate user!!  :O

I realized that I had to fugure out WHAT KIND of extension it was via the Ext. Mgr.  The Pure PHP Upload is a SERVER BEHAVIOR.  Once I saw that, I went to the S.B. window and figured out I had to first set up a teseting server.  After that I was able to use it.  Oh, wait, I had to "create an upload field" per the error message.

So, I guess I can say I am a "beginner" extension manager user.
