login authentication always fails

my login authentication page always goes to my login failed page no matter if i put the correct or incorrect username and password.

i have a problem making my login authentication page work. everytime i put the correct user name and login in the page it always takes me to the login fail page even though i followed every single step correctly on 4 or 5 different tutorials on login authenitcation for dmx. i have had this problem for close to a week now and would be really really grateful if anyone would know of some "hidden" or "between the lines" problem that would be causing my logins to fail each time i input the correct username and passwords.

Struggled with this for ages till I came across this.

Deleted all code automatically created by Dreamweaver and posted per article - and hey presto!
