A Web Form that dumps fields into a MYSQL database

I'm a beginner in PHP and Dreamweaver. I have created a simple form that allows Web visitors to add their names, addresses and answer a couple questions. I need a tutorial that shows me how to go about getting those populated fields from the form into the MYSQL database. Thanks!
