Upload File and Insert It into SQL Database

I would like to allow a user to upload a comma delimited text file to the website, and then have that file automaticaly inserted into a existing SQL table. The text file is a list of sales order transactions and include the following fields: OrderID, OrderDate, OrderAmount, CustomerID, CustomerName, Address, City, State, Zipcode, Phone, Email. Though the file structure is always the same, the uploader is often different, so the user's username would also need to be included in the insert. After the file has been inserted into the table, I would like the text file to be deleted off the web server. Finally, I would like the user to be directed to a results page indicating the size of the uploaded file, etc.
