Email completed document/form from web page

I'm not sure this is the right forum.I have a client who wants to put a Reservation Form on his website for customers to fill out and then just email the completed form from the web page.
After three weeks of searching and testing of different applications I have come to the conclusion that this is not feasible at this time. Or, did I miss something?
All I could find were expensive applications or forms which only emailed the individual elements, like Adobe Acrobat does.
Does anyone have any recommendations?

this is a fairly common requirement, there are a number of ways of attacking it, take a look at 'forms to go' - a small shareware applications (mac and pc versions) that will do exactly what you want.
You can do it with formmail too but I don't recommend it, as it doesn't look so good.
I did exactly what you are looking for at the end of last year, for a restaurant, reservation form asking for client details, land-line and test to prevent automated use, pm me and I'll send you a link - I don't want to put it here to prevent false traffic to his site <img src=../images/dmxzone/forum/icon_smile.gif border=0 align=middle>
