Pure Upload PHP - Error

Just purchased the extension, an am trying to upload an image and insert the filename into MySQL.So i follow the tutorial, create the insert recrod behaviour first and then add teh Pure Upload
And then i get a error and it buggers up the page. This is the error:
"While executing analyzeserverbehaviour in insertrecord.html, the following javascript error(s) occured:

TypeError : formNameReference has no properties."
Any ideas?
Annoying as I need to upload the location to a db.
I have deleted the DW cache file, but still doesnt work

It appears that you use third-party Dreamweaver extension for Insert Records.
And this extension is raising the error.
Unfortunately, DMXzone.com does not provide support for any third-party extensions.
To fix the problem try to disable this extension and use the standard Insert Record Server behavior of Dreamweaver.
If you have any future problems with Pure PHP Upload please write to the support forums of the extension:

Problem was i was using the same form i had created whilst trying out other methods of image uploading in DW, and that had its own post action, this was screwing things up.
