! Insert Record error: could not find form fields

I'm having a problem with the Insert Record feature of Dreamweaver MX 2004. I get the red exclamation point in front of the Insert Record entry in the server behaviors list. It does seem that my data is successfully being transferred to my Access database, but the error message is annoying and I worry that it will cause other problems down the road.

The only article I've found that seems to directly address this error is kb.adobe.com/selfservice/documentLink.do?micrositeId=MS_Customer&externalID=tn_16551 which only suggests that every field must be uniquely named. I've checked everything and that is not the case.

I think I've isolated the problem, however and I wonder if there is a remedy or if I'm just doing something odd. I am using <div>'s and a css file to format my form. If I give the <div> an id, then I get the exclamation point, and it claims that it can't find the form fields wrapped in that <div>. I use the id's both to stuff the formatting into the css file, and also with javascript functions to hide or unhide certain layers depending upon a users selection. Besides, it throws the same error whether I use and id and a css, or whether I put the formatting inline in the tag.

Am I doing something wrong, or is there some way around this?
