I'd like to create a Form within DW that can do the following:

When a user fills the form out, I plan to have an option where they will select , based on cities, areas where they wish to advertise their business.

For example:

A user will select MI as to where they wish to advertise, then they will select which type of advertisement they want to utilize.....then they will select a demographic, based on age range, and from that selection a list will then be populated with available mediums they can use for their criteria.

The form should, also, based on City, automatically list all stations within that city which should be triggered once they select City and then a further filtering once they select their demographic criteria.

Also, once the form is completed, the end user will view a summary page of all their crtieria. Preferrably, not mandatory, is to have a summary of items selected in a summary / paragrah, for example :

Thank you JOHN for visiting US...you have selected your advertising for MI and want RADIO Advertisement for 18-24 demogrpahic using WJZZ as your venue choice.

All items in uppercase will be those pieces that are to be filled in based on what the end user selects while completing the form.
