Whenever I upload large files this error message appears in a pop up window and the upload does not work.
The attempt to load 'Acccessing URL:www.mysite.com/? & GP_upload=true' failed.

On the server side my host admin has assured me that they have changed defaults to 25MB max size and 15 min timeout and that the problem in in the code (i.e. the Pure PHP Upload code) and that they can take a look at that code for $150/hr.

Note: small files, up to approx 7 or 8 MB work fine. Database gets updated and the contents of the form get emailed to me.

Please help. I have no hair left to pull out.

I think your problem can be caused by server-time-outs.
I´ve had the same problem - and gave up...
It´s got something to do with settings determined by your hostingcompany.
