Uploading file but not inserting record into DB

Hi i have applied the insert as well as the upload behaiour. the upload works fine, the file goes onto the server, but the record fails to insert into the Access DB. Im using Dreamweaver MX and the version of the Pure ASP Upload is 2.17. does anyone have any ideas on this one? thanks for now.dan
