I have 3 sites on my machine that have used asp upload for at least a year now, and I added it to another page today, and I get

You don't have ADO 2.5 installed on the server.
The File Upload extension needs ADO 2.5 or greater to run properly.
You can download the latest MDAC (ADO is included) from www.microsoft.com/data

I have 2.8, and 3 other sites that work fine on that box. I'm also using the image resizer. Does anyone know what else causes this error? It's not inserting, or uploading the .jpg either (obviously).

YES PLEASE! I am having this problem too. This happened to me after I installed MX 2004 and re-installed the Pure ASP Upload extension.

I have also confirmed that my server does have ADO 2.8 and another site where this is not working has ver. 2.7.

Please provide a fix for this soon!

You should install the latest version 2.15 of Pure ASp Upload - all problems are solved there. Get it from www.dmxzone.com/go?1622
