Access...

Im having a hard time deciding how I want to do tables for this, can someone point me in the right direction? I want to develop an online appointment scheduler for an orthodontist office similar to this on found at the DMV website....

eg.dmv.ca.gov/servlet/gov.ca.dmv.foAppt.SvAppt

I have not yet decided how I would create the tables. I have a plan down for the actuall querries but just cannot figure the tables out. Can someone please help me out?
