Select box and a search field?

Ive a database with names and telenumbers and so on....the problem for me is how to write the Sql code. The dropdown box is holding for example telenumbers or mobilenumbers or names and in the search box will i write the "search" for ex 070-6555777and chosing the mobile in the dropdown box
Please help.......
