Well, there are a number of options, and it really depends on what DB Environment you are used to.
My preference is MySQL Front, which is a Windows app. You can download it (it's free) at www.mysqlfront.de
Another option, which seems to be fairly popular with Access users is Mascon (there are commercial and free versions available). You can find this app (again Windows) at www.scibit.com/Products/Software/Utils/Mascon.htm the free version allows you to save your queries and design reports, and it comes highly recommended.
Hope this helps

DOn't get confused. MyAdmin and PHPMyAdmin are entirely different products.....
PHPMyAdmin is indeed web based, but MyAdmin is a full blown Windows App, similar to MySQL Front.

There is a project, still in beta, available here on udzone, which is an DW/UD extension that lets you administer a mysql server (add tables, drop fields,...) right into dreamweaver.
At the moment, the project is still in a form of command extension, though, after a major overhaul of the project code, it is now designed as a DW Floater extension.
I'm still developping the Update Record feature, and then will release this new version.
You can download it right here : www.udzone.com/showDetail.asp?TypeId=4&NewsId=1911
Any comments or feedback are always much appreciated.
