i am looking for a script which does the following.

i have a form which is going to contain regular fields(name,email etc...) and 5 check box / radio btns.. .
when the user submits the form based on what he chooses(the 5 box,or btns).
the details of his submission is shown in a html page where 5 columns are created for the 5 values .

i have been looking for this everywhere but could not get it.

i tried writing also. but i could not get the table part.
